LUHSD Athletic Hall of Fame Nomination Form

Nomination Application Criteria

 

1. Nominee must be out of High School for 7 years.

2. Individual LUHSD Athlete applicants must have graduated from our district and

    meet at least ONE of the following criteria to be nominated into our program:

3. You MUST INCLUDE A PHOTOGRAPH (Individual Photo ~ when nominee was an

    athlete is preferable) (Team ~ team photo)

4. Team Applications must have the names of all athletes and coaches with current contact information.

5. NOMINATION APPLICATION DEADLINE: JULY 31

                a. Mail completed application to:

                              LUHSD Athletic Hall of Fame Selection Committee Chairman

                              20 Oak Street Brentwood, CA 94513

                b. EMAIL APPLICATION: luhsd.athletic.hof@gmail.com

                c. You may hand deliver application to any HOF Board Member.

 

   **Incomplete applications or applications with no photo will not be considered

      for nomination.

 

6. Please include press clippings, pictures or other items that verify or document your athletic honors, if available. You may duplicate application pages and/or attach additional information if needed.

         NOTE: If nominee is deceased, please use nearest relative's information

 

 

REQUIREMENTS TO BE CONSIDERED FOR NOMINATION

ATHLETE

  • Most Valuable Athlete/Senior Athlete of the Year

  • First or Second Team All-League Selection. Athletes in sports without All-League Teams/Awards must finish 1st or 2nd in their individual event(s) at the League finals

  • First or Second Team All-Area Selection. Athletes in sports without All-Area Teams/Awards must finish 1st or 2nd in their individual event finals at a North Coast Section qualifying or championship event

  • Qualify for & compete in a California State CIF Championship Event

  • Recipient of an athletic scholarship to a 4-year college or university awarded to the applicant while a student at any high school within our district.

 

COACH

  • Documented record, awards & accomplishments of you LUHSD Career as a coach.
    Optional: other school, college or pro.

  • Number of championships (League/North Coast/State), won/loss records and list of sports you have coached in our district

 

TEAM

Considerations include:

  • Undisputed League/North Coast or state championship (Indicate year)

  • Season record/league and over-all (List)

  • State/National level of recognition (Championships/ranking/honorable mention)

  • Top athletes and documentation of stats & honors​

 

LUHSD CONTRIBUTOR

Focus is on accomplishments and contributions within the LUHSD sports community.

  •  Community Leader may be selected individually or as a group.

Family

  • Multi Family Members who have accomplished the above athletic and/or contribution criteria that graduated and/or been involved in the LUHSD athletic department
